What are your hours of Operation?
Our office hours are Monday through Thursday from 8:00 am-06:30 pm, we are closed on Fridays. We are closed on the following holidays: New Years Day, Martin Luther King's birthday, Memorial Day, Fourth of July, Labor Day, Thanksgiving and Christmas. The office does have an on call service during closed hours.
What insurance do you accept?

To ensure that you get the maximum benefit from your insurance provider, we offer the following tips for your consideration:
If you belong to a PPO or an HMO that requires you to see a selected primary care physician, please make sure that you have selected Family Practice Partners, LLC or one of its physicians before you are seen at one of our facilities. If you present for services with a card that does not state our facility or physician, your visit will be delayed while our insurance department verifies coverage with your insurance company.
If you have not received your insurance card before presenting it to our facility for treatment, you will be asked to pay in full, or if you know what your co-pay amount is, you may pay that amount. You will be responsible for your bill in full until you bring the insurance department a copy of your insurance card. We will not be able to bill your insurance company until we receive the card and complete billing information is on file.
Since we are primary care physicians, we will coordinate any referrals needed between your selected physician and your insurance company. Our referral department is familiar with your insurance company and contracts provided and will assist you in every effort when a referral is required. If you are a new patient and need an emergency referral, you will be asked to fill out new patient information in order for us to complete the referral process. All referrals must be approved by your selected physician and approved prior to any specialist visit.
If you do not see your insurance plan listed here, please contact your insurance company to inquire as to participation status. We may be participating providers contracted through another network. If we are not participating providers but you have out of network benefits, we can file your insurance for you. However, you will not get the network discounts and your benefits will be paid at the out-of-network rate.
If you have questions as to specific procedures performed in our office, you may contact our office for further information. If you have questions as to whether the services will be covered by your plan, you must verify benefits with your insurance company. Our office will not be responsible for claims denied due to policy provisions.
All insurance claims are filed electronically, if eligible. All other claims are sent on HICA 1500 paper forms. Turn around time for insurance processing usually ranges from two weeks to 60 days.
Do you accept walk-ins?
We strongly encourage patients to call and make appointments because due to the physicians' hectic schedules walk-ins may not be able to be seen that day.
How do you handle prescription refill request?
If you need a prescription refilled, we ask that you let us know at the time of your visit. We encourage you not to wait until the last pill to let us know that you need refills. If you call and want your prescription refilled via phone, please allow 24-48 hours for the prescription to be called in. Please check with your pharmacy prior to calling the office. This office has a very strict policy on narcotic medicines, we believe in pain and recognize that only the person experiencing the pain knows exactly how bad they feel. Unfortunately, narcotic pain medicines have a very high abuse and addiction potential. We do not call in or refill narcotic prescriptions on the weekend. Only the physician that initially wrote the prescription can re-write the narcotic prescription for you. There are DEA regulations that govern the dispensing of narcotics and this office abides by all DEA regulations. If you have any questions as to which medications are narcotics please do not hesitate to ask your doctor.
How are payments/billing handled through your office?
Your co-pay is due prior to your visit. We will directly bill your insurance company for your services. You are responsible for your deductible and insurance denials. If you have an outstanding balance, payment in full will be needed prior to the next visit. We do attempt to run this business in the most professional manner possible and appreciate your cooperation.
If I have test done, how will I obtain my results?
Most blood draws are done in the office. It takes on average 7-10 days for lab results to come back. The physician attempts to inform the patient of their results within 48 hours of his/her receiving the results. Patients are informed either by mail or phone, so it is imperative that we have a correct phone number and address at all times. It is the policy of Family Practice Partners, LLC that we do not discuss test results with anyone other than the patient except in the case of minors. We recognize that there are times one can become very anxious about test results. We as your physicians like to handle your results personally and not have the nursing staff do that job for us. This allows for better communication between the doctor and the patient.
